The Center has age, weight, last donation date, and travel criteria. You must also be in good health and not be taking any medication on the Medication Deferral List.
For patients who have been referred for donation because of a specific medical diagnosis, certain forms (as appropriate) must be competed by your physician.
After filling out this form, a registrar at our facility will call you back within one business day to confirm a date and time.
Special requests can be made regarding designated donors, autologous donors and/or therapeutic phlebotomy.
