PLEASE NOTE OUTPATIENT INFUSION CENTER GUIDELINES:

1. The referring physician is responsible for obtaining the consent on the hospital consent form prior to the transfusion.
2. No emergencies.
3. No patients who require a stretcher or have active infections (including active hepatitis, active C. Dificille or active tuberculosis).
4. No patients who are combative, actively vomiting, or require total nursing care.
5. No children under the age of 12 years, unless approved by the Outpatient Infusion Center’s Director at (212)434-2510.
